This form must be submitted fifteen (15) working days prior to the event; call five (5) days prior to confirm or cancel; 570-385-6090. Failure to notify the Business Office of an event cancellation less than 48 hours prior will result in a $50 set-up fee.

Please Note:

Entrance to Conference Center is located in front of building
Smoking is not permitted in any University building
Visitors must park in the Student Parking Lot, located to the left of the main drive
Non-University groups are responsible for their own set-ups and may be charged a fee for equipment and technician use.
Non-University organizations may request use of Campus facilities a maximum of four (4) times per year.
